If you are submitting grades from Fall 2024 for approved courses - Please send those grades BY JAN. 31, 2025 

Please read all instructions carefully...

• Application must be completed online in one session. If you leave the page, the information you have entered will not be saved.
  

• Online applications will only be accepted from September 2 until 3pm on September 30, for the class entering Fall 2025. Application will close at 3 pm on Monday September 30. 
• Please complete all required fields before submitting your application. If you accidentally submit an incomplete application, please call the Nursing Office at (831)646-4258 for instructions. 
• Official transcripts must be received in the nursing office by the close of the application cycle on September 30,  2024, in order for your application to be considered complete.
• If you submitted transcripts during a previous application cycle, you do not need to re-submit unless you have taken additional coursework for the application.

You must read this entire Application Packet before you submit your application. (It's a pdf document and is not contained in the form below). Forms to be submitted for multi-criterion points are in this packet. Please refer to the scoring tool on p. 24-27 and submit the required documentation to the nursing office by 3 PM September 30,  2024.  

Official transcripts must be sent directly to the Nursing Department. Students may submit unofficial transcripts for courses taken at MPC. 

(Note: please do not send transcripts to Counseling).
